CANCELLATION/RESCHEDULE/NO-SHOW POLICIES FOR ZAHRAZA ONCOLOGY ASSOCIATES, PLLC

THE PRACTICE

MYiDocUSA LLC provides management services for ZahRaza Oncology Associates, PLLC (the “Practice”).  MYiDocUSA LLC does not provide professional or clinical services.  All professional and clinical services are furnished by the Practice and providers employed or contracted with the Practice (each a “Provider”).

CANCEL/RESCHEDULE

Please remember that if you need to cancel or reschedule, please do so at least 24 hours in advance of your scheduled appointment using the link sent to you in your email. You will be responsible for a late cancellation fee of $100 if cancellation is less than 24 hours. This is necessary because a time commitment is made to you and is held exclusively for you.

For example, if you schedule appointment for January. 30, 2024 at 12 pm (Your local time). In case you need to cancel or reschedule, please do so at least before 12pm of Jan 29, 2024. If you cancel or reschedule after 12pm of Jan 28,2024, there will be a late cancellation fee of $100.

LATE TO THE VIRTUAL APPOINTMENT

If you are late for a session, you may lose some of that session time.

NO-SHOWS

A failure to show up for your appointment will be considered a “no show” and will be completely non-refundable. No-shows prevent other patients from booking appointments, affect their continuity of care and delay their access to care.

REFUND

To request a complete refund, you must cancel and request refund for your scheduled appointment at least 24 hours in advance of your scheduled appointment. To request refund, please email admin@MYiDocUSA.com. If request to refund is less than 24 hours before scheduled appointment, You will be responsible for a late cancellation fee of $100.

We reserve the right to suspend your MYiDocUSA and the Practice account or terminate your use of MYiDocUSA and the Practice services, if there are frequent no-shows, cancellations or reschedule of appointments within one year of time frame.
