MYiDocUSA LLC provides management services for ZahRaza Oncology Associates, PLLC (the “Practice”). MYiDocUSA LLC does not provide professional or clinical services. All professional and clinical services are furnished by the Practice and providers employed or contracted with the Practice (each a “Provider”).
The Practice does not accept insurance or participate in any insurance networks or reimbursement programs and is not able to offer services to Medicare or Medicaid beneficiaries at this time. The Practice requires payment at the time of service and will not bill insurance or accept reimbursement from insurance or the Medicare or Medicaid reimbursement programs.
Please remember that if you need to cancel or reschedule, please do so at least 24 hours in advance. You will be responsible for a late cancellation fee of $100 if cancellation is less than 24 hours. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you may lose some of that session time.
The Practice cannot ensure the confidentiality of any form of communication through electronic media, including text messages. If you consent to receive electronic communications from MYiDocUSA or the Practice, please know that such information may be lost, damaged or subject to unauthorized access. While we ask Practice Providers to return messages in a timely manner, we cannot guarantee an immediate response and request that you do not use these methods of communication to discuss diagnosis or treatment.
If you are a minor, your parents may be legally entitled to some information about your treatment. We will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. We may terminate treatment after appropriate discussion with you and a termination process if we determine that the treatment is not being effectively used or if you are in default on payment.
